The periorbital area or zone around the eyes is the focus of facial expression and communication. Subtle changes in these soft tissues create a profound change in how we are perceived by others. For example, laxity and drooping of soft tissues to the side of the brow and eye can convey sadness or fatigue, while depression in the central brow area suggests distress or hostility. Blepharoplasty or eyelid lift has long been an effective technique for correcting these problems. However, at the Hilger Clinic, we know that the eyes cannot be evaluated in isolation but need to be part of a treatment plan that embraces the entire region around the eye, if one is to achieve a refreshed, relaxed, approachable appearance. Thus, consideration of aesthetic surgery around the eye must include not only the eyelids, but also the brow/forehead and upper cheek areas. The eyelid skin is the thinnest soft tissue of the face, and laxity along with other aging changes will often manifest there first. An upper lid blepharoplasty, which is a minimally invasive procedure, can create a remarkable improvement.
Lower eyelid correction or lower lid blepharoplasty can greatly improve a tired appearance. The approach in our practice differs from the traditional fat removal surgery in that volume restoration to a deflated area is a routine consideration. The techniques we have developed employ less fat removal but improved lower eyelid contour by repositioning the fat in the adjacent hollows. This reduces the fat bulges and creates a youthful transition from eyelid to cheek.
The brow is essential in defining the eye area, and its position is always evaluated in our office before a treatment plan is developed with you. Improvement in the brow posture is achieved with a browlift or forehead lift. There are a variety of techniques available to accomplish this. Our office was the first in the area to adopt a less invasive endoscopic forehead lift. We presented a paper on this technique at a national meeting over 20 years ago and reported long-lasting correction and brow position, improvement in forehead wrinkles, and expedited recovery.
